Lower Blepharoplasty: A Lasting Solution for Under Eye Bags

As we age, one of the earliest and most noticeable changes often occurs under the eyes. Puffiness, bags, and a tired appearance can develop even when we feel well-rested. When patients come to me concerned about these changes, I often recommend lower blepharoplasty as a safe, effective, and long-lasting solution.

What Causes Under-Eye Bags?

Under-eye bags form as the fat that naturally cushions the eye starts to shift forward. Over time, the tissue and skin that once supported this fat weaken, allowing it to bulge outward. This process leads to the puffiness and shadowing that can make the eyes appear tired or aged.

How I Perform Lower Blepharoplasty

The procedure I use is called transconjunctival lower blepharoplasty. Through a small incision made discreetly behind the lower eyelid, I’m able to access and remove just the right amount of fat to smooth the contour beneath the eye.

This technique offers several important advantages:

  • No external incisions or visible scarring
  • Precise fat removal to reduce puffiness without creating hollowness
  • Natural-looking, long-lasting results

The key to success is achieving a balanced improvement—smoothing the under-eye area without overcorrecting and maintaining the natural structure of your face.

What About Dark Circles or Hollowing?

Not every patient has bulging fat under the eyes. Some patients experience dark circles or hollowing instead. In these cases, autologous fat transfer is often the better approach.

Using a very small amount of microfat—typically harvested from the abdomen, thighs, or flanks—I can gently fill the tear trough area. This restores volume and smooths out depressions under the eyes, helping to brighten and rejuvenate the overall appearance.

Recovery and Downtime

Recovery after lower blepharoplasty typically takes about 10 to 14 days. Mild to moderate swelling and some bruising are common, but these effects resolve steadily over the first two weeks. Most patients feel comfortable resuming social and professional activities after this recovery window.

Best of all, lower blepharoplasty offers permanent correction of under-eye bags—helping you look more rested and youthful for years to come.

Personalized Care

Every face is different, and so is every under-eye concern. During your consultation, I’ll evaluate whether lower blepharoplasty, fat transfer, or a combination of treatments will deliver the best and most natural outcome for you.
